When removing from teams, you will only see the team's listed that this employee is already a member of.






After this form is submitted:

  1. Approval will be requested of the team leaders / commander.
  2. Notification email will be sent to [email protected] distribution group.
  3. Active Directory changes will be queued to take place on the stated effective date.
